I guess it depends on what your current set up is. If you're running sendmail, you can just plug in the clamav-milter, and run it that way. If you're running exim3, you can either run it as it's own transport or use a backport of any of the above. If your an exim user, I would highly recommend A Metzler's backports of exim4 - they contain the exiscan patch to allow you to run clam at smtp time, 550'ing virus emails.
