I have checked the archives and got a tip that sendmail may not have installed libmilter even though I compiled and built it with the appropriate flags. So I manually ran ./Build ./Build install in the libmilter src and it is now installed. I checked the configure logs and the appropriat flags appear to have been found:
checking libmilter/mfapi.h usability... yes checking libmilter/mfapi.h presence... yes checking for libmilter/mfapi.h... yes checking for __gmpz_init in -lgmp... Yes config.status: creating clamav-milter/Makefile Yet after a make, make install still now clamav-milter...
