I'm running the stable release of clamav on a fairly heavy duty (20k mailboxes) mail server without any significant problems. Clamd seems to lock up periodically (a couple of times a week), but this isn't really a big deal, jsut means mail backs up a bit before our cron job notifies us and we just restart the process, after which it continues to chug along happily.
