We too had some trouble with the ClamAV milter on our servers, we went to MailScanner and it does the job nicely. (Though I would rather have a Non-Perl solution, we can wait)

It's configured to run as root at the moment, but
unless I can get it working fairly soon, I may
give amavis a whirl instead... :)
