You should definitely use Matt Simerson's qmail multiple patch: http://matt.simerson.com/computing/mail/toaster/qmail-1.03-toaster-2.3.patch
It is actually just a well-prepared set of other patches, without modification. It includes: SMTP AUTH QMAILQUEUE STARTTLS for qmail and more... I am using qmail + the patch above + qmail-scanner + clamscan + SMTP autentication, works fine. Just have to test clamd + clamdscan instead of clamscan...
