Oh and of course the TOC entries are real hyperlinks now, which only
underscores how much we're going to want to add support for user-driven
internal hyperlink macros.  For example, the leftmost table entries in the
"Document control settings" section of _ms.ms_ could hyperlink to the
subsequent section (or, with user-specifiable anchor locations, paragraph)
that presents the register, string, special character.

By God, we'll get _ms_ modernized yet.
