If anyone else encounters a similar problem, we narrowed it down to the SAS 
card. We were using a Dell PERC 5/E in the Gen 3 PowerEdge 2950 server running 
Debian 6, and later Debian 7.

As a workaround, we have installed Bacula on another server with PCI-X based 
SAS cards that use mtx to load an unload the library perfectly, and mounted our 
existing backups using NFS.

We have ordered a PERC 6/E dual port SAS card, if this resolves the problem I 
will update the thread accordingly.
