It happened to me quite a few time because of a power problem in one datacenter which was cutting power to our tape library. I just updated the slot status with the mtx command outside of bconsole to make sure that the status matches what bacula thinks.
I think unloading the tape in the slot with "mtx unload <sourceslot> <destslot>" and update again through bconsole should solve the problem.
