Sounds like dying hardware. Try to eliminate hardware/software. Stop your storage director and load a blank tape. Try writing zero's and random to it ("# dd if=/dev/random of=/dev/nsa0 bs=32k"). Try "mt erase", "mt rewind", "mt retension" -- ensure that all of the basic functionality still works.
