Might be hardware related, so it's useful to have a close look into 
your systems log.

Also, as re-starting the SD is definitely no fun when you're in a job 
already running for hours, I'd suggest to do the following the next 
time, if the first attempt to mount fails:
- watch the system log
- explicitly unmount the drive
- eject the tape
- load another tape
- try to mount that
- if that does not work, clean the drive
- if mounting a labeled tape still does not work, load a blank tape 
and try writing/reading it with tar or dd
- if that does work, but Bacula can't use the tape, use 'setdebug 
storage=VXA-2 level=400 trace=1', try to mount the tape again, and 
look at the SD's trace file. Perhaps there's an indication what 
exactly fails.

I'm rather sure that this is a hardware- or driver-related problem, 
though. It might be related to the use of a RAID-optimized SCSI HBA; 
those are typically expressy NOT recommended for tape drive attachment.
