I can't speak to _all_ mail systems, but the Maildir format doesn't have any indexes inherent in it. Most IMAP clients create their own indexes for speed purposes. I know Dovecot is smart enough to notice if an index is out of date or corrupt and rebuild it as needed. Courier has special commands to rebuild indexes as required.
