I have no idea what the problem is, but my best guess is that you have 
something like the following:
1. A bad disk or track on a disk
2. A memory problem that caused a disk block to get corrupted
3. Some program that messed up a portion of a Bacula Volume.

The way I would proceed is to write one or more new volumes, then restore the 
data in them and see if the problem reoccurs and if the files are properly 
restored.
