This email serves to inform you that we are currently missing some important
information required by the government of Canada regarding your account(s).
As of December 8th 2015, we are required by law to submit your designated
beneficiary or beneficiaries to the government of Canada. You will have until
December 11th 2015 to fill in the documentation that we require to keep your
account active and in good standing.
To do so, please complete and submit the form attached to this email to
designate a beneficiary or beneficiaries for your Savings, RSP, TFSA or RIF
plans. You can also use this form to designate your spouse or common-law
partner as your successor annuitant for your RIF Plan or Successor Holder for
your TFSA Plan.
The documentation we receive will be processed within three (3) calendar days
unless special circumstances apply or additional review is required. You may be
contacted in the event that any additional information is required.
Please note that failure to complete and submit the attached form by December
11th 2015 will result in your account being frozen. All transfers, payment
processing, accumulated interests or use of your debit card would be suspended.
