This is a known problem, but I don't have a solution right now. It appears that sometimes (maybe always on some systems) the fifo status wait function unexpectedly fails where it shouldn't as the lights on the IO box show the firmware was correctly uploaded. The problem is that the card isn't fully initialized then (no alsa pcm or midi devices are created yet). Running hdsploader another time performs this initialization. I'll take a closer look at this. You don't have to be root to run hdsploader.
