We used to have everything behind NAT except for customers that paid for a
static IP, so we were essentially charging extra for a public IP. It was
also a static IP, so it wasn't quite the same as charging for a public
dynamic IP, but most people just needed it for game consoles, and they
didn't really care about it being static. There were a surprisingly large
number of customers that had no problem paying extra for it, if you told
them that's what they needed for their xbox to work right.

But it got rid of a lot of weird random problems and made my life a lot
easier when switched all of our customers to dynamic public IPs. It's well
worth just paying for the IP space to do it right, in my opinion. The
biggest problem we ran into using private IPs were game consoles, but there
was certainly the odd issue here and there with all those other things as
well. Not to mention things like websites blocking IPs because they see too
many connections coming from it... I imagine proper CG-NAT would help with
a lot of that stuff, but that wasn't a thing back then. Doing CG-NAT plus
dual stack IPv6 would probably also work pretty well.
