I noticed that the country was listed as 'Afghanistan' so I inspected the live site and noticed they should be uppercase. Link from the docs or an example would help. I was looking at the placeholder docs https://developers.google.com/adwords/api/docs/appendix/placeholders and noticed I have not included the CONVERSION TYPE ID so I suspect this is causing the issue.
