I really don't remember controller model, I set it up about 3 years ago. I just 
remember that I needed to reflush controller firmware to make it work in JBOD 
mode.

You should seriously consider checking, and probably updating, the firmware on 
all your Seagate ST3500320AS 7200.11 drives.  Version SD15 (as reported above) 
is a known bad firmware.

Try this script from

 to see if you can convert the sd11 and sd12 names into the cXtXdX style.

My hunch is you have controller compatibility or controller configuration 
issues.  Please describe what controller hardware you are using and how it is 
setup.  It looks like controller 2 is hiding the details of the drives from the 
operating system and assigning its own unique device IDs (e.g. 
s...@n5000c5000b9f49ef).  Is controller 2 in AHCI mode right now?  (During the 
initial installation, controller 2 should have been in AHCI mode not RAID or 
JBOD mode.  I wouldn't change the mode at the moment unless you are ready to 
wipe/rebuild.)
