Hi, It definitely seems like hardware-related issue as panics related to common tools like format isn’t to be expected.
Anyhow. You might want to start to get all your disks show up in iostat / cfgadm before trying to import pool. You should replace controller if you have not already done so, and RAM should be all ok I guess?
