Are you able to tell us more precisely what you did with this disk?
For example, exactly how did you "attach" the disk?  Maybe it was in
fact "added" as a non-redundant second vdev? 

Can you attach the output of zdb -l from one of the pool devices? 
zdb -h may reveal more accurately the history of what was attached and
how.
