They've taken pains to limit the amount of sync'ing so it's only one or two calls to fsync (on files in the queue subdirectories) per incoming mail (I forget whether it's one or two). One of their performance advices was to be careful of syslog because some implementations call fsync on every line logged which will add a couple more sync's per message received and halve performance. I'm sure qmail also sync's at least once per message received.
