The same thing happened to me until I was reminded to patch the Live
Upgrade packages on the existing system before doing the upgrade.
This fixed it:
# cd /mnt/Solaris_11/Tools/I*
# ./liveupgrade20 -nodisplay -noconsole

# cd /
# luupgrade -u -n snv99be -s /mnt
