DB creation missing from "Install and configure in glance" page

Bug description:
This bug tracker is for errors with the documentation, use the
following as a template and remove or add fields as you see fit.
Convert [ ] into [x] to check boxes:
- [x] This doc is inaccurate in this way: Instructions for creating
the Glance database stops at logging into mysql. From there it skips
to sourcing the .rc file.
- [x] I have a fix to the document that I can paste below including example:
input and output.
These parts are missing:
$ CREATE DATABASE glance;
$ GRANT ALL PRIVILEGES ON glance.* to 'glance'@'localhost' IDENTIFIED BY
'GLANCE_DBPASS';
$ GRANT ALL PRIVILEGES ON glance.* to 'glance'@'%' IDENTIFIED BY
'GLANCE_DBPASS';
$ quit;
