I'm really sorry but we don't know how to do this.
XMLmind Software is not the author of the DocBook XSL stylesheets
(http://docbook.sourceforge.net/release/xsl/current/doc/) and we really
don't master[*] this complex piece of code.
If, for our own needs, we wanted to add images to the cover page and to
the header, we would try to find the information in "DocBook XSL: The
Complete Guide", http://www.sagehill.net/docbookxsl/.
For example, "Chapter 11. Title page customization",
