Thanks for the quick reply! my interim response is an extract from Tony Self's 
DITA Style Guide:

"When referring a reader to a step in a task [...] you must always use the 
<xref> element, with the type attribute set to li. (The <li> element is the 
ancestor of the <step> element)"

So this is where the use of type="li" comes from, and I thought I had 
successfully followed the same instructions for other xref.

As for the question of XML Mind version, I have forwarded your (helpful, and 
entirely justifiable) observation to my manager: and reminded him (though it is 
not his fault) that it is now nearly a year since I submitted my update 
request. The request has been approved, but getting anyone to take action and 
make the download is ... let's just say, not easy.

Sorry but I cannot reproduce this issue using latest version of XMLmind XML 
Editor and its bundled XMLmind DITA Converter. See attached sample.

As expected, the generated Web Help contains: "As explained in step #*2*, 
you'll see that foo is in bar state." *2* is the text generated for the empty 
xref. See attached screenshot.

You probably made a mistake. Please double-check what you did. For example, 
please take a close look at the warnings reported by "Map|Check Map".
