This is not the case.
It looks more like something between Java FX and XXE; the converted documents
on the Web are all OK... If you do not have any hints, do not bother, I am
pretty sure it would work at your location if I sent you the DocBook file and
the movie file.
We can live with this if we cannot put our hands on the issue.

1) A sample DocBook 5.1 document showing the problem.
2) A sample MP4 file of yours referenced from this DocBook 5.1.
3) IMPORTANT: the result of pasting what has been copied to the
clipboard by the "Copy" button of "Help|About XMLmind XML Editor". See

Maybe inspecting the Java exception stack while XXE is running would give us
some additional clue...
The Java exception is in principle displayed in the styled view of XXE
next to the media icon, as depicted in attached screenshot (exception
caused simply by the fact that the referenced MP4 file is missing).
Therefore, I doubt that you'll see something if you start XXE with a
console. But if this is the case, it would be a great help to see the
full stack trace.
