No, you haven't understood how XMLmind XML Editor really works (no offense intented).
You are confusing "Insert After" and "Add". "Insert After" is really "Insert just after selected element" and not "Insert _somewhere_ after after selected element".
You cannot insert a section after the title of a chapter because there can be no section between the title and the first paragraph of this chapter.
You cannot insert a section after a chapter because only another chapter or appendix, not a section, can go after a chapter.
You need to select *last* paragraph of the chapter (or last child element, list, table, section, whatever it is) and then use "Insert After".
