In your case, FOP's image cache contains the earlier version of the graphic. If I'm right, this would be an Apache FOP bug, not an XXE bug. Exiting from XXE and restarting it may suffice to solve the problem. At worst, give a different name to the new version of the graphic and then update its reference in the DITA topic.
