without any "insert" element or attribute, then your custom toolbar
"A" will completely replace stock toolbar "A".

I'll try to find the names of the ones I've mentioned (File, Command
History, Edit, etc) in the documentation inside XXE_install_dir/doc/, I'll come
back to
you if I cannot.

You'll not find File, Command History, Edit, etc, in any document found in folder XXE_install_dir/doc/.

These menus and actions are not documented and will never be (simply because there is no real need for such documentation).

"XMLmind XML Editor - Customizing the User Interface", Introduction ( http://www.xmlmind.com/xmleditor/_distrib/doc/gui/introduction.html ) explains:

---
Resource /gui/app/Professional.xxe_gui

Specifies the default user interface of XMLmind XML Editor Professional Edition. By doing this, it also specifies the user interface parts which are common to all XMLmind XML Editor variants.

...

A copy of these files is found in XXE_install_dir/doc/gui/gui/. This, because using Professional.xxe_gui as a reference when creating custom GUIs for XXE is absolutely required.

This copy of Professional.xxe_gui is also found online:

http://www.xmlmind.com/xmleditor/_distrib/doc/gui/gui/app/Professional.xxe_gui

In a nutshell, in order to customize the GUI of XXE, you are required to read and understand the contents of Professional.xxe_gui. Fortunately, the contents of this is not that long and quite simple to understand.
