Dear Hussein, it was an interoperability problem between yours (very good!) and another program.
Namely, i created a docbook with the eDE package. Tired of <tags> after a couple of paragraphs, i went googling for a DocBok interface, found yours among many others (your own was the best one by lengths) and started editing that docbook with xxe, hence the problems. I don't have eDE installed anymore, neither the file that was issuing the problem: i suspected something, so i created an empty docbook with xxe and pasted into it all the previous contents and it's working fine since. You may try to download eDE from http://www.e-novative.de/en/software/ede.php, create an "article" DocBook, add by hand a couple of paragraphs, impot into xxe and try to render in any format. I didn't do anything else. The program is working really fine now: the Italian settings and folders are not an issue.
