An interesting idea, Zdeněk, but it did not seem to achieve anything (nor was an error reported).  I amended the first pass file to end :

\eject

\immediate \write 18 {XeTeX "Hoi An TA menu (combine pages)"}

\end

and nothing different happened when compared to the version lacking this amendment.  Did I, perhaps, get the syntax wrong ?  I have never used \write 18 before.
There does not appear to be an option in TLMGR to turn \write 18 on or off, nor an option to check whether or not it is enabled, so debugging the root cause of the problem is not easy.

Hi,

I have never tried but could not you use \write18 to run XeTeX from XeTeX? I am not sure whether it is supported.
