I removed my texlive Junicode font set (dated 2009) and installed the "secret" ones from the CVS below, and presto, the minimal example "sa\d{m}kalita" posted by Nathan worked perfectly. Saṃkalita, for the non-Sanskritists, means "added together, accumulated," as, I suppose, a letter and a diacritical mark.
