Are you invoking tlmgr as root, for example as 'sudo tlmgr ...?'
The correct sequence of updating should be:
sudo tlmgr update --self
sudo tlmgr update --all
The first step updates tlmgr itself, necessary to install new
procedures, settings, (library) versions. Then tlmgr is certainly able
to update TeX Live. Maybe you have run into a situation where the old
tlmgr fails to read the new setup...
