I do not see anything in the log that shows that the checkbox is being initialized.
I copied the variable statement and the checkbox statement from your code and changed the names and I still do not see anything in my log. My log looks like this... [022C:5B60][2014-12-10T16:51:35]w321: Skipping dependency registration on package with no dependency providers: Server I'd almost think that it wasn't using my theme file except that the option dialog displays with the checkbox and text in the theme file. I'm wondering why the EulaAcceptCheckbox is displayed twice?
