I discovered the problem. The service had run-time issues. Once I cleared up these problems, the installer began to work properly (i.e. removing the service from the service list upon uninstall). I guess for some reason it was marked invalid and it took the cs.exe command to force it out of the registry.

I'm thinking it's the same general problem that the service is slow to shut down, but I was thinking the service might just be slow to shut down whereas it might be more broken than that. If there's any tracing in that service it would help to turn it on see what's happening when the uninstall asks it to stop. Clearly if it hangs for some reason it won't be deleted. Is the service marked disabled at the end of the uninstall? Still running?
