The details on how to sign a bootstrapper is here http://wix.sourceforge.net/manual-wix3/insignia.htm. I have found you only need to sign the bootstrapper and not the MSI's as well.
I don't think you can have a custom manifiest file with a bootstrapper - there was a discussion about it a few days ago.
