I didn't want to reply as when I tried to do something similar I couldn't get it to work properly using the Extension & Verb element tags so I fudged it using RegistryValues to add keys to HKLM\Software\Classes. If you can't get it to work using the Extension element you could always try that instead. I think for me it works out the same as the docs say for non-advertised Extensions, Verbs add rows to the Registry table anyway & I don't want Advertised extensions but from the code you originally posted it looks like you do.
