is a question of operator priority:
It should look like that
<Custom Action="Start" After="InstallFinalize">
(INSTALLERROR = 0) AND Not(Installed)</Custom>
Your term means
INSTALLERROR=(0 AND NOT Installed)
and this is of course always false
