For other open source options (like dotNetInstaller - see http://www.codeproject.com/install/dotNetInstaller.asp) check the archives of this mailing list. To answer the question of what the bootstrapper is, as you correctly surmised it is a small application (which you certainly could code yourself, but don't have to) which handles installation of any prerequisites (such as the .NET runtimes) that cannot be encapsulated within your MSI file due to licensing or other issues.
