ItemTemplates contains zipped versions of new project items and ProjectTemplates contains zipped versions of new projects. ItemTemplatesCache and ProjectTemplatesCache contain the same files, unzipped. I'm not sure what process is responsible for unzipping them - you would think this would fail for non-administrative users because the folders are not writable by a limited user (by default).
I believe the intent is that you can simply drop a new ZIP file into ItemTemplates or ProjectTemplates to extend Visual Studio. You may need to run devenv /installvstemplates to do so. If these folders are missing, that might suggest that some previous installer that you've subsequently uninstalled has accidentally deleted them.
