When I create a new application - type name, press "create" - the fields are
cleared, the browser page is reloaded with a bright orange announcement at
the top of the page

Do you mean when you hit  RETURN again?

If I do this after creating an app, nothing happens --- that is, the forms
are cleared, there is no submit button selected (so "RETURN" does not
activate any selection).

Can you tell more about what you are doing, which submit buttons you are
pressing, what browser you are using, etc.?
