There are already unit tests covering the macip acls and they don’t cause the problem, so it is some trigger that makes this happen. If you are able to isolate the trigger which is required to make this happen, it will help a lot. Unfortunately I can’t give you a step by step way to isolate that trigger because it can be most everything potentially.
