Depends on if you are running a firewall or router in front of your VNC server. If you are, you'll need to allow the VNC ports to pass through the router and/or firewall and to the destination machine.
Connecting locally will not involve the firewall or router, but connecting from the internet most likely will. Also, be sure you have to correct public IP address for access to the machine from the outside.
