I have seen this happen a few times on one of our clients' sites, I think I've tracked the problem down to a sticky shift key. Not a physical problem with the keyboard however, but if you select files on the windows desktop for example it performs a multiple selection. It's almost as though VNC transmits the 'SHIFT key down' signal, but not the release.
