Dan, Further to Chris Johansen's email, change the NAT forwarding at your work to port 5900, use a VNC client at home to try connecting to the work PC. Make sure that port 5900 is also open on your home PC if you do not have a direct connection to the internet at home.
Since you have a DSL connection at work you might be able for testing purposes to use the IP address to connect from home. At work on the VNC server visit the following website http://www.whatismyipaddress.com/ and make a note of the IP address. At home using the VNC viewer type in the IP address you were given above, this should work as long as the DSL router was not rebooted between the time you got the IP address and the time you try at home (I also assume that port 5900 is open and the VNC server is using display number 0).
