I have seen this occur on win2k machine with newer versions of VNC. (release 9). I'm not exactly sure of the cause, but I know the version of r7 that I have does not do this.
By all that I mean I had the same symptoms with a win2k computer connecting the first time and fails to connect everytime after that. I replaced the executable with an older version, and the problems went away. Never had time to go back and figure out exactly what happened.
