Ah, but what if I have resized my VNC viewer so that it is smaller than the X server desktop? Then the edge of the VNC viewer window no longer coincides with the virtual screen. Or what if I use keyboard commands to navigate from the VNC viewer app to another process (easy to do on a Mac or PC)? Or another process grabs the focus away from the VNC viewer? In fact I usually see the cursor displayed by Xvnc sitting slightly inside the virtual screen after I quickly move the mouse out of the window....

It just seems impossible for the Xvnc server to detect anything about the user's remote client beyond what the VNC viewer explicitly sends it.
