I must agree with Bill.  I would like to add, that, while using dictation,
punctuation can be easily forgotten, as it is not natural to include when
speaking.  However, inserting punctuation is incredibly important, for both
screen-reader users, and those who read visually.  A lack of punctuation can
completely change the meaning of one's text.  Not to mention the impression
it makes on your reader.
For clarities sake, getting in the habit of inserting punctuation while
dictating is very important.  Yes, it takes some practice, but it is simple.
If using SIRI, for example, simply saying the word "comma", will insert a
comma.  The same is true for most punctuation symbols.  This includes the
words "period", "comma", "exclamation point", "question mark".  There are
some that take some finesse, such as adding quotation marks, but even
parenthetical insertions are easily done.  You don't need to indicate
capitals at the beginning of a sentence, since saying "period" will indicate
to SIRI that your sentence has ended, and she will automatically capitalize
the next word.

For those using their iPhone to send e-mail; might I suggest that you get in
the habit of hitting the "More/Numbers" button so you can properly punctuate
your messages.  I'm not being an English grammar cop here, trying to listen
to those messages that drone on and on without any punctuation is terribly
difficult when one is relying on a screen reader to receive that message. 
And I imagine there are a number of us that do indeed rely on screen readers
to receive our e-mail.
