Mary, This is a known bug for VO users. I've seen it myself more than once. This is how I get around it. I first tap the new playlist button. The name field is blank at this point and you can type whatever name you want. Next I tap the add music button and add the first song and any additional songs subsequently. Another thing to note is that as you type the name of the playlist you won't hear your letters echoed by VO.
Please report this issue to apple accessibility. Hopefully they will eventually fix it. I hope this helps.
