Very strange. My iPhone XR running iOS 15.6.1 is set to currently not speak caller ID but still does announce who's calling. This happens also when wearing headphones like Sony XM3 and Apple AirPods 3. gen.
I suspect VoiceOver speaks caller ID regardless of that setting since the screen activates and VO reads the new screen content or something. If I turn off VO the caller ID is not spoken. If I set it to always speak caller ID and turn off VO it works as expected and my phone then speaks caller ID.
