I've experienced this myself, but not on iOS 14.2. Not yet at least. The only fix I found to work was to reboot my phone and all well for a while and then VO suddenly stopped announcing incoming calls again. So, I started rebooting my phone more often to make sure everything works as expected. When upgrading to 14.2 I performed a reset to factory defaults (deleted all settings and data) and the problem of VO not announcing incoming calls seems to be gone now. I use the default banner style and announce incoming calls is set to always.
